Assessment, Counseling, and Coordination
This includes the professional assessment of care needs, the scheduling of visits, as well as counseling and coordination with clients, family members, doctors, and other involved parties.
Before it can be determined which services are needed and which costs will be covered, your individual support needs must first be assessed. That’s exactly what the needs assessment by SpitexCare is for.
During this process, we work together to determine which nursing, care, or support services are necessary for daily life. The needs assessment takes place at the client’s home and serves as the basis for the next steps with the primary care physician and health insurance provider.
The needs assessment takes place at the client’s home. A SpitexCare professional visits the client to gain a comprehensive understanding of their personal situation and determine what kind of support is needed in daily life.
The following are assessed, for example:
health status
Limitations in daily life
Care needs
psychosocial stress
Mobility and risk of falls
Medication status
Family Situation
Needed and Desired Support
Frequency and duration of visits

Therapeutic care encompasses medical and nursing services. These include, for example, preparing and administering medications, wound care, injections, and other medically prescribed procedures.
Basic care provides direct assistance with daily personal activities, such as washing, showering, getting dressed and undressed, moving around, eating and drinking, and using the restroom.
Psychosocial care supports people with mental illnesses or psychosocial stressors. The goal is to promote stability, independence, and security in everyday life.
Care provides support in daily life, fosters a sense of security, and promotes social participation. This includes, for example, companionship, conversations, shared activities, and assistance with daily tasks.
Housekeeping includes assistance with household tasks, such as cleaning the home, doing laundry, grocery shopping, or other daily chores.
When family members are heavily involved in caregiving, targeted respite care can free up time and help stabilize the caregiving situation in the long term.
Not all types of home care are funded in the same way. Under certain conditions, nursing care services may be partially covered by basic health insurance. Caregiving, housekeeping, or other support services, on the other hand, are billed differently and must be paid for privately or through supplemental insurance, depending on the situation.
For this reason, the needs assessment carefully documents exactly what kind of support is needed and which category of services it falls under. The necessary documentation for the family doctor and the health insurance provider is then prepared for the corresponding care services.
A SpitexCare professional visits the client at home to assess their individual needs for nursing care, assistance, and support. During this visit, they also determine which services are needed and how often visits should take place.
After the appointment, SpitexCare prepares the necessary documents and sends the corresponding form to the primary care physician for review.
The family doctor reviews the documents and confirms the necessary care services.
The documents are then submitted to the health insurance provider. Care services are initially approved for three months.
Once approval is granted, SpitexCare will begin providing the agreed-upon nursing and care services.
After three months, the need for care is reassessed. Based on this assessment, the required care services are adjusted. The subsequent authorization is valid for six months.
