Why is the needs assessment important?
The needs assessment is the first and decisive step in the care process - it forms the basis for individually tailored care from SpitexCare. The aim of the assessment is to determine the specific need for care, support and assistance in order to be able to offer customized services. This ensures that clients receive exactly the help they need - no more and no less.
In addition, a structured and professional needs assessment ensures that resources are used optimally and that a high quality of life and the greatest possible independence in everyday life are maintained.
The needs assessment process
The needs assessment is carried out by qualified care professionals with extensive experience. It takes place in the client's usual home. The personal interview enables a holistic assessment of the situation. The following aspects are taken into account:

This comprehensive assessment is the basis for planning and coordinating the required services.
Your care needs are then summarized in the "Needs assessment" form and we send it to your family doctor. The health insurance company will approve your needs in the first phase for a maximum of three months. A further needs assessment is then carried out for the following six months.
Finally, the family doctor reviews the necessity of the measures and approves them. We then ensure that the confirmed needs assessment is sent to the health insurance company.
What information is required?
Various pieces of information are required for a comprehensive needs assessment. This includes, for example

The role of relatives in assessing needs
Relatives play a central role in the assessment of needs. They know the person concerned very well and can provide valuable information about their day-to-day needs and challenges. Their involvement ensures that the care plan is realistic and feasible.
In addition, relatives can also support the implementation of the care plan and thus contribute to a better quality of life for those affected. Open communication between care staff, patients and relatives is therefore essential.
